Liability and Photo/Video Release Forms

All participants and observers will be required to fill out a liability form and photo release form. If a guardian will not be accompanying their participant who is under the age of 18 please pre-fill out each form and have the participant bring the completed forms with them to class. These forms are a requirement in order to take any Erie Dance Consortium class.
